Cochiti Pueblo is conducting a Community Health Assessment (CHA) to better understand the knowledge, attitudes, and behaviors of community members related to health and wellness. This survey has been approved by tribal leadership and is being conducted by the Cochiti Health Department, in partnership with the Albuquerque Area Southwest Tribal Epidemiology Center (AASTEC) and Albuquerque Area Indian Health Board (AAIHB). Your input will help identify health needs in the community and guide the development of services and programs to improve the overall health and well-being of our people.
All eligible tribal members (16 years and older) are invited to participate. The survey can be completed either on paper or electronically using a computer. It will take about 1 hour to complete and includes questions on a wide range of topics such as general health and medical care, chronic conditions, mental health and support, physical activity, nutrition and food access, traditional healing and culture, alcohol and drug use, community services, language, and financial well-being.
You are free to do the survey or not. If you don't want to participate at all, or if you don't want to answer a particular question, that's okay. If you choose to do the survey, your answers will be kept confidential. Your name will not be on any reports about this survey. Your answers will be grouped with those from other tribal members in the Pueblo.
