About this survey:

The Pueblo of Santo Domingo, the Kewa Pueblo HealthCorporation, and the Albuquerque Area Southwest Tribal Epidemiology Center(AASTEC) are conducting this community health survey to better understand your knowledge, opinions, and behaviors related to health and wellness. Your participation will help us identify community strengths and needs and improve our services for the community. The survey will take 20-40 minutes to complete. The survey includes a variety of questions about health, and some questions may be sensitive, such as questions about mental health, alcohol and drug use, and sexually transmitted infections. All tribal members 18 years and older who live in the Pueblo have been invited to participate in this survey.

Your rights:

You are free to do the survey or not. If you don't want to participate at all,or if you don't want to answer a particular question, that's okay. If you choose to do the survey, your answers will be kept confidential. Your name will not be on any reportsabout this survey. Your answers will be grouped with those from other tribal members inthe Pueblo.

Once we receive the completed survey from you, we will mail you a $25 gift card as compensation for participating in this survey. Additionally, we will enter yourname into a raffle for grand prizes.
